This cut is referred to as a "Square Emerald" The piece of rough from which this stone was faceted was mined about 40 years ago (circa 1976), and comes from the collection of Richard Kosnar, who had a strong presence in Brazil bringing back a large quantity of pegmatite minerals and gem material in the 1970s.
